AFSCME Council 5 -- Member's Health and Welfare fund is making a change from Delta Dental to HealthPartners effective January 1, 2019. Below is a new summary of the dental benefits.
Rest assured you will be able to continue to see the dentist you and your family are seeing today.
In the coming weeks, you will be receiving a packet from HealthPartners that provides specific information relating to the plan benefits and network providers. This packet will provide you with details and resources available to you with HealthPartners as our plan administrator. Open enrollment will take place from November 1 through November 21, 2018.
You should have received a letter in the mail with your current enrollment information. If that information is correct, you do not need to do anything -- your coverage will continue as is.
If you need to add or drop any dependents, please do so by logging onto the AFSCME Council 5 Memberlink: http://afscmemn.org/dental then go to Memberlink and complete the Dental Enrollment/Change Form. The effective date of any change will be January 1, 2019. All forms MUST be submitted no later than November 21, 2018.
